Failure Mode and Effect Analysis (FMEA) is a risk analysis tool that can be used to identify the points at which the process may fail. It is a structured method that helps identifying potential failure modes, what causes them, and their consequences. It then guides in taking actions to either prevent these failures or correct them. FMEA can be applied to manufacturing and non-manufacturing processes, and can be performed at the system, assembly or part levels.

This FMEA template is a Microsoft Excel spreadsheet that you can use and modify to suit your specific needs. It can easily be modified to include additional sections if desired. For example, you may want to add additional rows to include more process steps or product parts.

This FMEA tool should be seen as a living document, not something that you do once then store away. It should be continually updated as changes happen or more information is gained. This ensures your processes stay as robust and error-free as possible. Please help us to make this template better by providing us with your comments and suggestions.
